When we create a capture page, I don't see a box to check to have them put in their email address or phone number with their name. How do we put that in? All it shows is First and Last name.

The email address is requested automatically when you select a target mailing list, because it's always required. We are going to be adding the phone number option soon. We have a few other items further up on our "To Do" list, but it should be up by Friday.
